A survey was conducted in Tillsonburg identify community needs and challenges. The objective of the exercise was to promote the prioritization of the needs according to their urgency to approach assisting the community in the best way. Based on the surveys, the healthcare sector is the most pressurized regarding access to resources and care personnel. While most of the respondents reported that they were comfortable with various services, such as transportation facilities, housing, and the general environment, some registered concern about community healthcare. The percentage of the aging population continues to increase thus straining the available healthcare resources. Therefore, he expected the outcome of the project is to ensure that healthcare is accessible and affordable for all the residents of Tillsonburg.
An aging population is a unique group in the community of Tillsonburg. Most of those who live in Tillsonburg are retired and have no source of employment. The city is often marketed as a retirement city, and thus many retirees opt to purchase real estate and dwell in the area. Studying the concept of community improvement will require an effective choice of the target population because of its dynamic. Indeed, the choice to address the increase in this population and its impacts on the healthcare sector is well informed. Improving the quality of healthcare may enable the augmentation of national productivity; hence Tillsonburg offers a satisfactory basis for healthcare intervention. The change model for intervention selected for the project also reflects the needs of the community and is based on the characteristics of Tillsonburg hence it is capable of producing the intended outcomes. With members of the group being part of the general population of Tillsonburg, the community issues can be understood from a first person’s report perspective, which makes the interventions even more plausible.
Priority Issue, Aims, and Expected Outcomes
The priority issue in the case under consideration is the increase in the aging population, which strains the healthcare sector. The matter is of great concern to the community of Tillsonburg since it limits the quality of healthcare the people of the area receive. A community such as Tillsonburg with a large percentage of aging individuals should have multiple long-term care facilities that provide sufficient care to the targeted populations. The establishments could remove pressure from existing healthcare facilities, which serve the general community besides the aging persons. While conducting the community assessment, the objective was to explore the extent of the mentioned problem and come up with viable and sustainable solutions for the healthcare sector. The community assessment helped to identify the true needs of the community and the assets they have, which could help in addressing the healthcare sector challenges within the area.
The aging population of Tillsonburg is about 67 percent of the community, which implies that the portion that is produced is lower than 33%. Therefore, the city leadership will have to implement strategies that are both affordable and suitable such that they serve the aging people and the rest within the constraints of finances to enhance the quality of care. With or without federal support, the city should find the creative and effective use of the available assets for the development of strong healthcare systems. The transport sector, housing facilities, industries, and businesses are part of the sufficient and effective features of Tillsonburg. They can be considered as the community assets and thus be a foundation for the development of better healthcare infrastructures.
The project aims at improving the healthcare service provision in Tillsonburg. One of the objectives of the intervention would be to increase the density of healthcare facilities in the area by advocating for the creation of an additional healthcare facility in each of the districts of Tillsonburg within the next year. Increasing the number of healthcare facilities will reduce the ratio of patients to healthcare facilities. The key stakeholders needed to accomplish the objective include the government entities at the federal and state levels, private investors, and the local community. The provision of land and real estate can be assigned to the local community as the investors provide additional resources for expansion of the healthcare sector. The federal and state governments could provide support in the form of healthcare insurance coverage targeting newer healthcare facilities.
Another objective of intervention would be to initiate a long-term care project chain within Tillsonburg, which would address the specific needs of the aging population, the particularly there constant need for care within the next 2 years. Most of the aging population is vulnerable to health conditions such as pulmonary diseases, which require care. With long-term care facilities that are supported by the state, the needs of this group will be met. Moreover, the pressure on the existing shared healthcare facilities will be lifted resulting in an improvement of healthcare quality among the entire population. The intervention will be founded on the theory of community organization as described by Harris (2010) to ensure that it succeeds. The theory describes community organization concerning the concepts of community empowerment, critical consciousness, social capital, issue selection, community capacity, participation, and relevance. Regarding the issue at hand, the selection has been made through community assessment. The intervention process will entail the concepts in community organization. Participation and involvement of the community depend on the community’s understanding of the issues that surround it. With the help of the government entities, the Tillsonburg community will have sufficient social capital and capacity to address the challenges facing the healthcare sector.
Upon the implementation of the interventions, it is expected that health outcomes of the community will improve significantly. One of the expected changes will be in the quality of healthcare in Tillsonburg across all age groups. With reduced pressure on healthcare facilities, it is expected that the professionals will attend to fewer people per unit time. As a result, there will be a reduction in the waiting time per patient, and an increase in the attention accorded to each patient. For the aging community members, access to better care will be enhanced. Through the long-term care projects, the community will have most of their needs met, converse to what they experience in the conventional healthcare systems, which are not designed for the provision of long-term care. The bed space in the Tillsonburg healthcare sector will increase resulting in greater potential for in-hospital thus more efficient patient management. The overall outcome expected is that there will be increased productivity due to better health outcomes.
